explanation of laser machine settings

I recently got LHR using the Cynosure Elite Alex laser. I’m the curious type (borderline OCD/nerdy) and want to learn more. The settings used were:

Fluence - 18J
Pulse Width - 20
Rep Rate - 1.5 Hz

I understand that fluence is essentially power…the higher the Joules the higher the power. I also understand the relationship between spot size and Joules (i.e. larger spot size = more penetration and smaller spot sizes require higher Joules).

However, what I do not understand is the role that pulse width/duration and rep rate play??? I read on another site that the ideal pulse width for LHR is between 10-30ms.

Can someone explain? Thank you

BTW, I am white, skin FITZ 2.

You’re missing spot size but if I remember right, that model only has 15mm max spot size. So 18j on 15mm spot size is good setting but pulse width needs to be lower for treatment efficacy. Your skin type should handle lower pulse. Ask them to try 10 or lower pulse width. I’m skin type III and I was able to tolerate 3ms pulse at 18j 18mm on GentleLase machine. Pulse width is like measuring how large the wave is, the smaller the better. You can search this subject in the laser section here. Pulse width has been discussed before by laser tech.

I’m frustrated. The clinic was using the Candela Gentlelase before getting the Cynosure. On the Candela I was getting treated with 18mm spot size at 20J. The treatments were not pleasant. After my last treatment (with the Cynosure) I commented to the tech that it wasn’t near as painful. I then asked her if the settings were high and she said yes. I guess that’s not really true???

Candela GentleLase
Spot size - 18mm
Pulse Duration - 3ms (fixed)
Joules - 20 (max)

Cynsoure Elite
Spot size - 15 mm
Pulse duration - 20ms
Joules - 18

So I’ve done some more reading/research and if I’m understanding the variables correctly, there is NO WAY the settings on the Cynosure would be as effective as the Candela. Is this a fact?

I really want to nail this down b/c as I mentioned, I was being treated with the Candela using the above settings and was switched to the Cynosure (above settings) and now in hindsight feel like my last appt. might have been a waste.
